I think that maybe what you’re looking for is a “nav log” or “trip sheet”. Several commercially produced ones are available but I’ve never found one that I really liked (as you say, too much useless stuff) so I made one up to suit my requirements. I just print more pages when I need them then hole-punch the pages so they can be inserted in a small 3-ring binder that I keep in the plane. I use one of those soft cover binders (velcro cover, not zipper, its easier) that you can get at places like Office Max, Staples or Walmart because it also holds a note pad, credit cards, pencils and other useful stuff. Figure out what your boss needs on a trip sheet and make one up for him. Send me an email address and I’ll send you a copy of what I use. This should help you get started.

Just head on out to the local pilot shop at any airport and see what they have to offer.
Then again, you could make your own log pages, suitable for your own purposes, head on off to Kinko’s, print up a bunch of pages and have ‘em spiral bound.
